APPOINTMENTS​

• Please call the office during office hours to schedule regular appointments.

• You must call to schedule both regular (well child visits) and sick visits. 
• Forms are usually completed free of charge when presented during a well child visit. Form completion at other times is subject to a fee.

AFTER HOURS/EMERGENCIES 

• Call 911 and transport your child to ER for any life threatening emergencies at any time.​​
• Please call either of our office numbers 215 722 2022 or 215 526 1893 after office hours for urgent matters only.

REQUESTS FOR RECORDS

Fees for requests for medical records will be charged per currently applicable guidelines of the Pennsylvania Department of Health.
